Implicaciones del registro REACH en medicina interna

ResumenLa enfermedad cardiovascular ateroesclerótica sigue siendo la principal causa de morbimortalidad en todo el mundo. Aún se desconoce alguna de sus características esenciales, aunque se conocen muchos factores de riesgo que favorecen su desarrollo. Por otro lado, no se tienen datos precisos acerca de las características de los pacientes que tienen más riesgo de presentar un nuevo episodio aterotrombótico. Con el objetivo de paliar estas deficiencias se puso en marcha el registro REACH (REduction of Atherothrombosis for Continued Health). Los resultados del estudio REACH confirman la necesidad de entender la aterotrombosis como una enfermedad global y no individualizarla según el lecho vascular afectado. Desde el punto de vista del internista, los aspectos más interesantes que hay que reflejar son: reclamar un abordaje multidisciplinario de la enfermedad, con el concurso de varias especialidades médicas y quirúrgicas; por su carácter integrador el internista debe ser pieza fundamental en este equipo; la solución a esta pandemia de mortalidad cardiovascular no la tiene únicamente el personal sanitario, sino que deben implicarse tanto las administraciones públicas como la sociedad civil.

Atherosclerotic cardiovascular disease continues to be the main cause of morbidity and mortality worldwide. Some of the essential characteristics of this disease are still unknown, even though many risk factors are well established. In addition, there are no exact data on the characteristics of patients at greatest risk of a new atherothrombotic episode. To fill these gaps, the Reduction of Atherothrombosis for Continued Health (REACH) registry has been established. The results of the REACH study confirm the need to understand atherothrombosis as a global disease and not to individualize it according to the diseased vascular bed. From the internist's point of view, the most interesting features are the following: the need for a multidisciplinary approach to the disease, with the participation of various medical and surgical specialties; due to their integrative perspective, internists should be a key figure in this team; the solution to this pandemic of cardiovascular mortality does not lie solely in the hands of health professionals but also requires the involvement of both public administrations and civil society.
