Abstract
Women with pre-existing hypothyroidism who become pregnant should increase their levothyroxine dose around 25-30% in early pregnancy. When hypothyroidism is discovered during pregnancy, levothyroxine treatment should be initiated to restore euthyroidism as soon as possible. Subclinical and mild overt asymptomatic hyperthyroidism during pregnancy can be monitored with no treatment. Moderate to severe hyperthyroidism requires treatment with the lowest dose of thionamides able to maintain mother's free T4 in the high level of the normal range.
