Abstract

ResumenEl dolor torácico tiene una enorme variedad de causas, algunas de las cuales requieren mayor urgencia en su abordaje que otras. Uno de los orígenes a tener muy en cuenta en atención primaria es la cardiopatía isquémica, ya que presenta una alta morbimortalidad prehospitalaria.Presentamos el caso de un paciente que acude a su centro de salud por malestar general con decaimiento y artromialgias en ambos brazos.Realizamos un ECG en el que se aprecia fibrilación auricular rápida y elevación del ST en la cara inferior. Tras comprobar que el resto de constantes vitales se encuentran dentro de la normalidad, se deriva al paciente al servicio de urgencias del hospital de referencia.Allí ingresa directamente en la UCI. Tras realizarle un cateterismo, es diagnosticado de infarto agudo de miocardio con clasificación funcional de Killip grado i y enfermedad de 2 vasos (coronaria derecha y arteria bisectriz).

Chest pain can be due to a wide variety of causes, some of which require greater urgency in their approach than others. One of the origins to take very much into account in Primary Care is ischaemic heart disease, since it has a high pre-hospital morbidity and mortality.The case is presented of a patient who went to his Health Centre due to feeling generally unwell with weakness and muscle and joint pains in both arms.An ECG was performed, which showed rapid atrial fibrillation with ST elevation in the lower side. After checking that rest of the vital signs were within normal, the patient was referred to the Emergency Department of the reference hospital. From there he was admitted directly into the ICU. After performing a catheterisation, he was diagnosed with an acute myocardial infarction with Grade 1 Killip functional class, and 2-vessel disease (right coronary and bisecting artery).
