Los cambios en el paciente desde el punto de vista médico

ResumenComo sanitarios asistimos al progresivo envejecimiento poblacional que conlleva cronicidad y discapacidad y que precisará no tanto prevención o curación, sino cuidados y mayor coordinación. En el debate actual entran en juego corresponsabilidad con la salud, participación en el diseño y gestión del sistema sanitario, alfabetización sanitaria y la excesiva medicalización de la vida. La inmediatez se ha convertido en urgencia médica y esta inmediatez no está exenta de riesgos. Se constata la transcendencia de saber explicar, escuchar y tranquilizar en los encuentros sanitarios. Internet comienza a facilitar el acceso al historial clínico, trámites administrativos, facilita encuentros no presenciales, provoca cambios en los comportamientos de salud pero a su vez desinformación, ansiedad, expectativas alejadas de la realidad y desigualdades. Saber conciliar los fines de la medicina ante los nuevos entornos y escenarios sanitarios descritos en este texto facilitará el equilibrio entre expectativas futuras de salud que desearíamos conseguir y las posibilidades presentes.

As clinicians we witness the gradual ageing of the population that entails chronicity and disability and that will require not only prevention or curing, but also care and better coordination. The current debate is where joint responsibility with health, participation in design and healthcare management, health literacy and excessive medicalization come into play. Immediacy has become a medical emergency and this immediacy is not without risks. The importance of explaining, listening and reassuring during health encounters has been noted. Internet is beginning to facilitate access to clinical history, administrative procedures; it promotes changes in health behaviours, but also misinformation, anxiety, unrealistic expectations and inequalities at the same time. Reconciling the motivations of medicine when it comes to new health environments and scenarios described in this text will facilitate the balance between future health expectations of what we would want to obtain and present possibilities.
