ResumenEl tratamiento preventivo del ictus isquémico se basa en la actuación sobre los factores de riesgo vascular, así como en medidas terapéuticas específicas según el subtipo etiológico de ictus. En el ictus isquémico de origen aterotrombótico se indicará un tratamiento antiagregante, estatinas y procedimientos de intervención carotídea en casos seleccionados con estenosis de arteria carótida ipsilateral. En los de causa lacunar, tratamiento antiagregante y estatinas. En los de origen cardioembólico, tratamiento anticoagulante (el tipo de agente anticoagulante y la intensidad de la anticoagulación varían en función de la fuente cardioembólica). En el ictus isquémico de causa inhabitual, el tratamiento preventivo dependerá de la etiología. En el ictus criptogénico, antiagregación plaquetaria, recomendándose además la monitorización del ritmo cardiaco prolongada en busca de fibrilación auricular paroxística.

Preventive treatment for ischemic stroke is based on actions taken on the vascular risk factors as well as the specific therapeutic measures according to the etiological subtype of the stroke. For ischemic stroke of atherothrombotic origin, the indication is for antiplatelet therapy, statins and carotid interventional procedures in selected cases with ipsilateral carotid artery stenosis. In lacunar stroke, antiplatelet therapy and statins are indicated. For stroke of cardioembolic origin, the indication is for anticoagulant treatment (the type of anticoagulant agent and the intensity of the anticoagulation vary according to the cardioembolic source). For ischemic stroke of uncommon cause, the preventive treatment will depend on the etiology. For cryptogenic stroke, antiplatelet therapy is recommended, along with extended cardiac rhythm monitoring in search of paroxysmal atrial fibrillation.
