Abstract
Muscle pain is a non-specific symptom, very common in the normal population. It is most frequently due to metabolic, endocrine, toxic or infectious conditions. Myalgia is not a common symptom in primary muscle conditions, however, metabolic myopathies and some other myopathies can present with muscle pain. Myalgia due to primary muscle conditions is usually related to exercise and other symptoms or signs are present, such as weakness, muscle atrophy or hypertrophy or elevation of the CK. In patients with metabolic myopathies due to abnormalities in the glycolytic pathway symptoms will arise after brief and intense exercise. Fatty acid oxidation defects lead to muscle pain and other symptoms after prolonged exercise, or in association with fasting or febrile illnesses. It is important to consider ischemic pain among the differential in exercise-related muscle pain due to its different management and implications.
