RésuméObservationUne patiente âgée de 74 ans, aux antécédents de carcinome transitionnel papillaire de l’urètre traité par curiethérapie et radiothérapie, a été hospitalisée pour l’exploration d’une ascite chyleuse. Le bilan étiologique a conclu à un chylopéritoine par brèche lymphatique post-radique. Un traitement conservateur comprenant un régime pauvre en graisses et riche en protides, associé à des triglycérides à chaîne moyenne, a permis l’assèchement de l’ascite.DiscussionLe développement d’une ascite chyleuse après radiothérapie devrait faire rechercher en premier lieu une récidive tumorale. Cependant, l’origine radique doit être évoquée, car son évolution est habituellement bénigne avec une bonne réponse au traitement conservateur.
