MRI evaluation following partial HIFU therapy for localized prostate cancer: A single-center study

SummaryObjectiveTo evaluate the value of MRI for surveillance of primary hemi-HIFU therapy for localized PCa in a single-center.Patients and methodsPatients with localized prostate cancer were treated with hemi-HIFU from October 2009 to March 2014. All patients performed MRI before focal therapy, the reader was blinded to the treatment. Oncological failure was defined as positive biopsy or biochemical recurrence (Phoenix).ResultsTwenty-five patients were treated with hemi-HIFU in one center. The median nadir PSA was 1.45 ± 1.4 ng/mL. Prostate volume decreased from 45 cc to 25 cc on MRI findings. At 20 months, none of the patients had histological recurrence. Biochemical-free survival rate was 88%. MRI evaluation had a negative predictive value of 100% on the treated area and 81% on the untreated area. PSAd ≥ 0.1 ng/mL2 was a predictive factor for cancer on untreated area (P = 0.042).ConclusionMRI control at 6 months is a potentially effective evaluation of treated area after hemi-HIFU and may replace randomized biopsies if PSAd < 0.1 ng/mL2 during follow-up.Level of evidence4.

RésuméObjectifÉtudier la fiabilité de l’IRM dans la surveillance du traitement par hémi-HIFU dans les cancers localisés de prostate.Population et méthodesÉtude monocentrique, d’une cohorte de 25 patients consécutifs ayant eu une séance d’hémi-HIFU pour cancer de la prostate localisé, entre octobre 2009 et mars 2014. Les critères d’évaluation étaient : PBP de contrôle à 6 mois, suivi du PSA total et réalisation d’une IRM de contrôle. L’ensemble des IRM pré- et postopératoires ont été relues par un seul praticien, en aveugle du traitement avec recherche de cible, évaluation du volume prostatique et description des modifications IRM induites par HIFU.RésultatsVingt-cinq patients ont été traités par HIFU centré sur un lobe dans notre centre. Le volume initial moyen mesuré par IRM a été réduit de 45 mL à 25 mL. Le PSA nadir médian était de 1,45 ± 1,4 ng/mL. Apres un suivi médian de 21,2 mois, aucune récidive sur la zone traitée n’a été prouvée. L’évaluation IRM de la zone traitée avait une valeur prédictive négative de 100 % et de 81 % sur la zone non traitée. Un PSAd ≥ 0,1 ng/mL2 était un facteur prédictif de tumeur controlatérale (p = 0,042).ConclusionL’IRM de contrôle à 6 mois après traitement par hémi-HIFU a montré son efficacité dans la surveillance de la glande traitée et pourrait se substituer aux biopsies randomisées en cas de PSAd ≥ 0,1 ng/mL2 au cours du suivi.Niveau de preuve4.
