Décès maternel après rupture spontanée d’une artère utérine dans le post-partum immédiat : à propos d’un cas

RésuméNous rapportons le cas d’un décès maternel survenu après la rupture spontanée d’une artère utérine dans le postpartum immédiat. La patiente a présenté des douleurs abdominales et un collapsus une heure après un accouchement normal. La présence d’un hémopéritoine a conduit à une laparotomie qui a mis en évidence un saignement intrapéritonéal provenant de l’artère utérine droite. Une ligature de l’artère utérine et une hystérectomie d’hémostase ont été réalisées. Une coagulopathie de consommation s’est rapidement installée entraînant une défaillance multiviscérale et le décès de la patiente 18 heures après l’accouchement. Il s’agit du premier cas rapporté de décès maternel lié à une rupture spontanée d’une artère utérine.

We report the case of a maternal death occurring after spontaneous rupture of a uterine artery immediately following delivery. The patient presented abdominal pain and a collapsus one hour after a normal delivery. Laparotomy revealed massive haemoperitoneum and intraperitoneal bleeding from the right uterine artery. Ligature of the uterine artery and hemostasis hysterectomy were performed but the patient died of multivisceral failure 18 h after the delivery. This is the first case report of maternal death occurring after spontaneous rupture of a uterine artery.
