Prophylaxie des endophtalmies après chirurgie de cataracte

Dans cette revue, nous analysons les éléments les plus récents en matière d’antibioprophylaxie lors de la chirurgie de la cataracte. Ce domaine a bénéficié d’une vaste étude mise en place par l’ESCRS (European Society of Cataract and Refactive Surgeons) et publiée récemment ; l’injection intramcamérulaire de céfuroxime à la fin de l’intervention permettrait de diminuer le taux d’endophtalmie après chirurgie de la cataracte par un facteur de cinq. À partir d’une recherche bibliographique large, nous donnons les informations qui sont fondées sur la médecine basée sur les preuves. Enfin, nous livrons des éléments pratiques en associant les mesures d’asepsie et d’antisepsie pour une prévention globale des endophtalmies survenant après la chirurgie de la cataracte.

In this review, we report the most recent findings on antibiotic prophylaxis during cataract surgery. The ESCRS (European Society of Cataract and Refractive Surgeons) has published the results of a study using a cefuroxime injection in the anterior chamber at the end of cataract surgery. It was found that this antibiotic injection reduced the rate of postoperative endophthalmitis fivefold. Based on the analysis of the available literature, we summarize the relevant information consistent with evidence-based medicine. Finally, we provide additional practical tips on antibiotic prophylaxis such as asepsis and antisepsis to decrease postcataract endophthalmitis.
