Réaction à corps étranger dans les rhizarthroses traitées par trapézectomie et interposition d’un « anchois » en acide L-polylactique (Arex®615R). À propos de huit cas

RésuméLa rhizarthrose est une affection fréquente. En cas de capital osseux insuffisant pour la mise en place de prothèse totale ou en cas d’atteinte péritrapézienne, les auteurs ont utilisé une trapézectomie avec interposition d’un implant résorbable en acide L-polylactique (Arex®615R). Cette technique est simple et rapide. Entre 2006 et 2010, sur 68 implants posés, neuf ont été à l’origine d’une réaction inflammatoire prolongée, anormale sur le plan clinique et radiologique, qui a conduit à en pratiquer l’ablation avant la fin de la troisième année postopératoire. L’histologie a mis en évidence dans tous ces cas une réaction immunitaire résorptive gigantocellulaire à corps étranger.

Osteoarthritis of the trapeziometacarpal joint is a common pathology. When the trapezium is not large enough to allow using a total joint arthroplasty or in case of peritrapezial osteoarthrosis, the authors used a trapeziectomy with interposition of an absorbable L-polylactic acid anchovy (Arex®615R). This technique is simple and fast. From 2006 to 2010, out of 68 implants, nine displayed a prolonged inflammatory reaction, both clinically and radiologically abnormal, leading the patients to undergo revision surgery for removal of the implant before the end of the third postoperative year. Histological analysis highlighted in all the cases a resorptive gigantocellular immune foreign body reaction.
