Transferts osseux vasculaires libres (TOVL) « en bout de chaîne »

RésuméL’os vascularisé a prouvé sa capacité à consolider, en l’absence de résorption et dans des milieux scléreux, voire septique. La reconstruction distale d’un doigt est souvent difficile d’accès pour un greffon pédiculé et une greffe conventionnelle se résorbe. Ainsi, les transferts osseux vascularisés libres (TOVL) trouvent leur indication dans les reconstructions distales de doigts après séquelles d’amputation traumatiques. En fonction de la perte de substance à reconstruire, l’os vascularisé est prélevé avec de la pulpe, de l’appareil unguéal ou du cartilage de croissance chez l’enfant. Nous rapporterons notre expérience de la technique de wrap around modifiée, de transferts partiels aux dépens du deuxième orteil et la technique de reconstruction d’ongle en griffe. Le transfert vascularisé du cartilage de croissance chez l’enfant est essentiel pour permettre la croissance de la phalange transférée. Les indications sont rares cependant les enfants, les musiciens ou les patients très motivés dont le métier nécessite des gestes fins peuvent bénéficier de ce type de microchirurgie reconstructrice sophistiquée.

The vascularized bone proved its capacity to be healed, without resorption even in sclerotic or infected tissue. So the free vascularized bone transfer (FVBT) is indicated in distal post-traumatic finger amputations. According to the loss of substance to be reconstructed, the vascularised bone is harvested with some pulp, the nail complex or the cartilage of growth for a child. We report our experience of the technique of wrap around modified, of partial transfers of the second toe and the technique of reconstruction of claw nail deformity. The vascularised transfer of the cartilage of growth at the child is essential to allow the growth of the transferred phalanx. The indications are rare however the children, the musicians or the patients who require fine pinch can benefit from this type of sophisticated reconstructive microsurgery.
