Abstract
An individualized plan of care is critical for managing the patient with fracture nonunion. Optimizing the host and the local biomechanical milieu are key elements for treatment success. Mindful preoperative planning is paramount. Atrophic and oligotrophic variants frequently require both mechanical stability and biologic supplementation. Deformity correction is integral in the algorithm.
