Abstract

Transgenicular osteotomy is an appropriate operative treatment for the correction of malalignment of the knee joint in unicompartment arthritis and instability. The evolution of operative techniques and hardware designs have led to a safer procedure and, therefore, to a wider range of indications. However, there is still a high failure rate following osteotomies when the biomechanic and biologic environments are not considered with adequate respect. The most common cause of failures and pitfalls and their solutions are described.
