Reconstruction du radius distal par la méthode d’Ilizarov après résection d’une tumeur à cellules géantes: À propos d’un cas

RésuméUn cas de résection en bloc de l’extrémité distale du radius pour tumeur à cellules géantes est rapporté. La perte de substance osseuse de 8 cm a été traitée par la technique de l’ascenseur avec le fixateur externe d’Ilizarov, après corticotomie du fragment proximal du radius. Une reconstruction satisfaisante avec néo-articulation radio-carpienne a été obtenue. Le poignet a récupéré une fonction satisfaisante. Aucune récidive n’a été observée à 5 ans de recul. Les arguments qui ont conduit à ce choix thérapeutique sont évalués.

We present a case of Ilizarov reconstruction after en bloc resection of the distal radius. An 8-cm resection was required to remove a giant-cell tumor. Reconstruction was based on the ascension technique using an Ilizarov external fixator after corticotomy of the proximal fragment of the radius. Satisfactory reconstruction with formation of a neoarticulation between the radius and the carpus was achieved. Satisfactory wrist function was achieved and the patient was recurrence free at five years. The arguments leading to this therapeutic option are discussed.
