RésuméL’incidence de la tuberculose a fortement baissé depuis l’obligation de déclaration et la vaccination généralisée au milieu du xxe siècle. De nombreux pays ont suspendu le caractère obligatoire de la vaccination, préférant la réserver à des populations à risque. La France avait conservé la vaccination généralisée obligatoire, utilisant une injection intradermique dont la réalisation est difficile et les effets secondaires nombreux. Depuis 2004, différents avis sont favorables à une vaccination réservée aux populations à risque, en particulier celles originaires de pays à forte endémie de tuberculose. Ces avis préconisent également de renforcer le dépistage des sujets atteints de tuberculose. Madame Roselyne Bachelot, ministre de la Santé, a annoncé le 11 juillet 2007 la suspension du caractère obligatoire du BCG chez l’enfant et l’adolescent au profit d’une recommandation forte de vaccination des enfants les plus exposés à la tuberculose dès le 1er mois de vie. Parallèlement, un programme national de lutte contre la tuberculose a été lancé pour une période de 2 ans (2007–2009).
SummaryThe incidence of tuberculosis strongly dropped since the obligation of declaration and the vaccination generalized in the middle of the 20th century. Many countries suspended the obligatory character of vaccination, preferring to reserve it to populations at risk. Mrs Roselyne Bachelot, Minister of Health, announced on July 11, 2007 the suspension of the obligatory character of the BCG from the child and the teenager with the profit of a strong recommendation of vaccination of the children most exposed to tuberculosis as of the first month of life. In parallel, a national programme of fight against tuberculosis 2007–2009 is launched.
