Abstract
With significant proportions of young people reporting a chronic illness in addition to improving survival rates for many childhood onset conditions, the provision of transitional care has become the focus of attention for clinicians, policymakers and researchers alike. The aim of this paper is to move beyond the realms of philosophy, so extensively documented to date, and to explore the evidence base and challenges of putting transition into action.
