Abstract
Nidotherapy describes the systematic manipulation of the environment in all its forms so that a better fit is created between the patient and his or her surroundings. It is not directed at changing the patient but as it involves a close relationship with the patient to evaluate all their environmental needs -social, physical and personal- it can be looked on as a therapy rather than a technological exercise. It is particularly useful in the management of those with personality disorders that are treatmentresisting, which includes the majority, as in this population much is blamed on the environment and little on the patient. nidotherapy requires considerable flexibility, initiative and the ability to make empathic relationships, as the therapist needs to understand the environmental needs through an implicit understanding of the patient's normal functioning and modus vivendi and often involves an advocacy role. It is normally administered by a single therapist working under supervision and with close liaison with other services, and initial data suggest a treatment period of 1 year.
