Abstract
Grade 3 lung cancers represent an heterogeneous group in terms of surgical indication. In elderly subject, an oncogeriatric assessment with evaluation of the vulnerability is essential, through scales as the Balducci classification before any therapeutic decision. Surgical resection followed by chemoradiotherapy is recommended in early stages with preserved health condition. In the most vulnerable subjects or with later stage, treatment requires sequential chemoradiotherapy, monochemotherapy, or isolated radiotherapy. Supportive cares are essential, particularly in this fragile population.
