Abstract
COPD patients are frail subjects frequently presenting with various co-morbidities that further increase the already quite high operative risk due to the impairment of lung function. The indication for surgical resection for lung cancer further worsens the prognosis given the high risk of severe disability after surgery. A thorough preoperative assessment of these patients is warranted to choose with each of them the best treatment option for a decent quality of life, a compromise between an impairment of lung function and a poor prognosis of lung cancer if not operated. Lung function tests at rest and exercise, as well as quantitative lung perfusion scans can target the operative risk in order to evaluate, on a case by case basis and within a multidisciplinary team, the benefit risk ratio of the surgical or another therapeutic alternative.
