Abstract
Low back pain in an adult achondroplast presents a diagnostic and therapeutic challenge. Congenital, degenerative and acute etiologies often co-exist, and complicate management decisions. MRI plays an integral role in the diagnostic pathway but potential pitfalls do exist. We illustrate this with a case where Modic changes at a single vertebral level mimic an acute fracture on MRI.
