Abstract

The CAST incident analysis technique is applied to the 2005 Buncefield incident when a fire and explosion due to the overfilling of a gasoline tank caused catastrophic damage to a key piece of national infrastructure. Causes are identified at every level from the equipment up to corporate management. The analysis provides new insights into how the incident was caused and how it could have been prevented, which were not highlighted in official reports. These insights are widely applicable to all organisations managing major accident hazards.
