•Anterior process fractures have never been documented in nursing literature.•Accounts for approximately 15% of all calcaneal fractures.•Mechanism of injury and the clinical presentation is similar to that of an ankle sprain.•Increase awareness when viewing radiographs.•This fracture is easily missed and are commonly misdiagnosed as ankle sprains.
