Abstract
The intraoperative period is not a black box and the occurrence of adverse events during surgery is associated with a worse postoperative prognosis. The cornerstone of high-risk cardiac patient intraoperative management is to maintain myocardial oxygen balance and its major determinants within surgery. A simple multimodal approach can significantly reduce both postoperative troponin release and long-term major adverse cardiac events. The surgical Apgar score might be of great interest to accurately determine intraoperative adverse events and facilitate the choice of postoperative clinical pathway in high-risk cardiac patients.
