Abstract
Primary care is deeply concerned and impacted by the development of chronic diseases especially diabetes, cardiovascular diseases, cognitive disorders, other complex medico-social situations. This requires structuring a better care path and organizing into health clusters or pluri-professional health centers that can better meet the needs of this new medical era.
