Abstract
⺠The application of chronic disease management recommendations in epilepsy care is reviewed. ⺠Chronic disease management models recommend patient education and self-management. ⺠The epilepsy specialist nurse can play a vital role in patient education as well as coordination and monitoring of care. ⺠Primary care clinicians with appropriate education support, incentives, and clinical guidelines can become expert managers of well-controlled epilepsy. ⺠Integration of care across organizational boundaries should be facilitated by information and communication technology.
