► Epilepsy surgery is an effective treatment for intractable partial epilepsy. ► Favorable candidates include patients with lesional epilepsy and medial temporal lobe epilepsy. ► There is conflicting evidence regarding the efficacy and safety of surgery in non-lesional extratemporal epilepsy. ► The controversy surrounding epilepsy surgery in patients with non-lesional epilepsy will be reviewed.
