Abstract
The development of hospitalisation at home (HAH) in France is recent with the first opening in 1957 at the Assistance Publique-Hôpitaux de Paris (AP-HP). Currently, the epidemiological transition is associated with an increase in the number of elderly patients treated in HAH with often complex needs. Partnerships between prescribers and HAH and coordination of interventions need to be strengthened to meet the needs of these elderly patients. Improving the partnership between hospitals of the AP-HP and the HAH was based on a collaborative work after an inventory. The objectives of this collaborative work were to better formalize the admission addressing and to improve the patient's pathway. In addition, improving the professional coordination at home has been defined throughout the pre-admission, the home-care and the preparation of the HAH discharge. This professional coordination relies on the assessment of patients' needs, the care planning and the diffusion of the geriatric culture in HAH.
