► Tractor roll-over deaths in Victoria, Australia fell significantly between 1985 and 2010. ► Mandatory roll-over protection retrofitting did not have a simple, direct impact. ► New tractor fitment, rebates and publicity prior to regulation had a cumulative effect. ► During the same period, an increase in run over fatalities occurred. ► A similar systematic prevention strategy is needed for run over deaths.
