Abstract
⺠We extend the current literature on complexity theory and health care by drawing on the dissipative structures model. ⺠We provide rich empirical data about a self-organized transformation process in Sweden spanning 19 years (1992-2011). ⺠We criticize the common focus on episodic innovation and illustrate the potential of ongoing, practice-driven change.
