Abstract
The diagnosis of the iron deficiency is difficult among. patients reached of chronic inflammatory diseases. The usual biological markers of the martial statute are in fact influenced by the inflammatory reaction. It is in this context that the blood determination of the soluble receiver of the transferrine (RsTf) is principally indicated. During this prospective study, we determinate level of RsTf within three groups of patients. The first group corresponds to the pilot subjects (n = 20). They are all male and 18 to 22 years olds. The second group included female patients from 20 to 45 years olds and having a ironless anaemia ,(n=20). No one of these patients receive a iron substitutive therapeutic. The third group corresponds to the patients hospitalized in the internal medicine service for chronic inflammatory pathologies (n=12). The RsTf is determinates by using N RsTf Latex reagent on BN Prospec automat of the Dade Behring company. The index of RsTf is also calculated. The pilot subjects have a rate of RsTf and RsTf index which join the reference values given by Dade Behring. For the patients with iron deficiency anemia of the second group, the rate of RsTf is increased (average: 2,86 mg/I) just as the index of RsTf (average: 3,21). The patients of the third group are subdivided to two groups according to the rate normal or raised of RsTf and the RsTf index. At the latter, the martial deficiency could not be established by ferritin (wrongfully normal or increased). These results show the interest of the determination of RsTf in the diagnosis of the martial deficiency among patients hospitalized for chronic inflammatory pathologies. In the absence of any inflammatory syndrome, the determination of ferritin is the examination of first intention to carry the diagnosis of iron deficiency.
