Abstract
Patient requiring a parenteral nutrition at home must be treated by a multidisciplinary et pluriprofessional team at the different stages of a “care pathway” designed to improve autonomy and security, as recommended in existing guidelines. The care pathway in PN has five stages: (1) the initial assessment leading to inclusion in home parenteral nutrition program, (2) hospitalization for implantation of the vascular access, (3) initiation of parenteral nutrition, (4) organizing the return to home and monitoring the parenteral nutrition at home, (5) management of complications and hospital care for severe complications. The description of a patient's care pathway requiring parenteral nutrition shows the importance of coordination between the hospital (home parenteral nutrition unit) and community care actors involved, and need to make the patient actor of his own care, through therapeutic education program.
