- Spontaneous coronary artery dissection (SCAD) is increasingly recognized as a cause of ACS in young and middle-aged females.
- SCAD of the left main coronary artery is rare.
- Intracoronary imaging with OCT can facilitate the definitive diagnosis of SCAD, but is not without risks.
- Conservative management of SCAD appears to be associated with good clinical outcomes in the absence of ongoing ischemia.
- The optimal treatment of SCAD in the left main coronary artery remains unknown.
