Abstract
Care of the patient with acute type A aortic dissection requires a breadth of critical care knowledge and surgical techniques. The surgical approach is often dictated by patient-specific anatomy. For cases where the dissection involves the aortic root, this complex anatomical region must be addressed with close attention to the valve apparatus and coronary artery origins. In the vast majority of patients, repair of the aortic root is our preferred approach vs replacement. A reproducible technique for repair of the dissected aortic root is described, which utilizes a neo-medial layer of felt, limited surgical adhesive administration, valve re-suspension, and external felt reinforcement of the anastomosis. General patient considerations and key technical points are highlighted.
