Abstract
A borderline left heart may be present in patients with hypoplastic left heart syndrome or an unbalanced atrioventricular canal defect. Traditionally, such patients have been managed either by high-risk biventricular repair in infancy or during the neonatal period, or by definitive single-ventricle palliation. In this article, we present an approach for staged left ventricular recruitment that promotes gradual rehabilitation of left heart structures and allows delay in the commitment to either a single-ventricle pathway or a biventricular pathway. The rationale, techniques, and early results are described.
