Abstract
The concept of Director of Medical Emergency Care (DSM) must be revisited in light of the recent terrorist attacks committed in France and abroad. The implementation of the emergency individual and collective health care pathway is the cornerstone of the management of victims of mass casualty situations. This medical strategy from the site to the hospital can only be defined and implemented by a physician whose daily work it is and who simultaneously masters treatment and stabilization techniques, medical dispatching, and the healthcare available in the area and region. The SAMU Medical Director is the hospital-based physician, who brings together all of the necessary skills and tools. In the field, the strategic decisions must be taken under the authority of the prefect, by a triumvirate of the law-enforcement (police-gendarmerie) commander (COPG), the commander of rescue operations (firefighters-rescuers) (COS), and the coordinator of medical operations (COM), a SAMU physician.
