Abstract
Therapeutic patient education (TPE) of the diabetic persons can be proposed in the city with the private practicing health professionals (HPs) who are closest to patient's reality life. To do this, the private HPs must agree to work together, on the basis of a common reference frame, and organize the patient education pathway. The diabetologist-trained to diabetes pathology and trained in TPE appears to be the best candidate to coordinate this pathway; however, the organizational difficulties of urban medicine and the working habits of private practicing doctors limit his actions. In the future, the private HPs will have to work as a team if they want to product quality TPE with efficient coordination. But medical codification also has to be rethought because it is a major limitation for the development of TPE in private medicine practice.
