Abstract
Our country is facing a phenomenon of aging and ramping up of chronic diseases such as type 2 diabetes and patients with several diseases. This is accompanied by considerable health costs and a need for major organizational changes. These organizational changes place primary care at the center of the healthcare system. They emphasize networking and ambulatory approaches in general, including therapeutic education of patients to dealing with comorbidities. The world of health is changing radically and the profession of diabetologist - hospital-based as well as private pratice - must be actively associated with these developments. It must dynamically contribute to them, or it may lose its purpose at the detriment of patients.
