Abstract
Colorectal surgeons are often involved in the multidisciplinary management of young patients with endometriosis. Endometriosis, which can involve all the organs of the pelvis including the uterus, ovaries and fallopian tubes, has the potential to cause infertility. Complete excision of all lesions of endometriosis should not be performed at the risk of irreversible destruction of organs necessary for procreation, and permanent infertility should not be the price to pay for improving the painful symptoms caused by endometriosis. Surgical treatment of ovarian endometrioma should be tailored according to the patient's wishes for future conception and to the preoperative ovarian reserve. For patients who do not desire immediate pregnancy, continuous use of birth control pills to suppress the ovarian cycle is recommended. Surgery for deep pelvic endometriosis is followed by a high rate of successful conception and pregnancy, and it does not seem to diminish the chances for successful in vitro fertilization (IVF).
