Quelle place pour les médiations thérapeutiques dans la « psychothérapie par l'institution » ?

RésuméLes médiations thérapeutiques sont une partie intégrante du traitement par l'institution que nous proposons dans le cadre de la clinique Dupré. Elles contribuent à adapter ce cadre aux caractéristiques psychopathologiques des adolescents et jeunes adultes qui y sont traités. Elles représentent une technique pour se lier, alors même que le lien, qui est notre principal instrument thérapeutique, est aussi ce qui menace le plus nos patients. Elles permettent d'élaborer les répétitions en utilisant les capacités empathiques des soignants inclus dans l'espace psychique élargi du patient. Ce faisant, elle contribue à une bonne intrication des mouvements de liaison et de déliaison si fortement mobilisés chez les patients adolescents et jeunes adultes. Au travers d'un trajet de soins, nous tenterons de montrer la place et les enjeux thérapeutiques des médiations dans notre dispositif institutionnel. Cette démarche nous conduit à définir un point de vue plus général sur la « psychothérapie par l'institution » et la place qu'y occupent les médiations thérapeutiques.

Therapeutic mediations are an essential part of the communautary treatment offered by our Clinic (Clinique Dupre de Sceaux). They contribute to the adaptation of our therapeutic program to the psychopathological characteristics of our adolescents and young adults patients. We see this therapeutic mediations as a tool to allow a bounding between the treating team and patients whose central psychopathological feature is the fear of bounding and relationship with significative others seen as potential threat of dependency and/or seduction. Through therapeutic mediations, the treating staff is able to deal with repetitive symptomatic situations, using the empathic capacity of each member of the team included in the “psychic enlarged space” of the patient. Therapeutic mediations are the contributing to regulate the bounding process in this type of patient. Through a clinical presentation this paper will present the use of therapeutic mediations in our Clinic. We will then discuss more widely the concept of “psychotherapy through the institution” we use in this type of therapeutic setting.
