L’hospitalisation sous l’angle psychodramatique ou « l’incasable Monsieur B. »

RésuméÀ l’aide du récit des évènements entourant l’hospitalisation d’un patient nommé Monsieur B., les auteurs tentent de reconstruire ce qui, du passé non symbolisé de ce patient, va se rejouer lors de cette période régressive. L’observation et le décodage des mises en scènes institutionnelles suscitées par le patient dans l’environnement hospitalier le révèlent bien comme un « utilisateur » du lieu dont il fait usage, souvent à son insu, en produisant une reconfiguration singulière de ce qui est institué et constitué comme modèle d’accueil et de soins. Ainsi, les auteurs passent-ils en revue cette reconfiguration de l’institution soignante en observant les différents moments dans lesquels une dynamique interactive spécifique est impulsée entre le patient et d’autres patients mais aussi avec le corps soignant et sa hiérarchie. Ils proposent l’idée du déploiement d’un scénario fantasmatique primaire propre au patient et perceptible dans les mouvements et les ressentis des professionnels, dans les agirs de ce patient et dans ses dires lors d’entretiens duels. Les éléments traumatiques connus de la vie du patient sont mis en rapport avec les scènes produites dans l’espace institutionnel. Ces mises en correspondance éclairent comment l’hôpital peut être le théâtre d’une simple répétition des traumatismes ou bien se constituer en espace propice à leur élaboration partielle.

Drawing on the story of events surrounding the hospitalization of a patient named Mr. B., the authors try to reconstruct what, from the non-symbolized past of this patient, is going to be replayed during this regressive period. Observation and decoding of the institutional mise-en-scene that the patient evokes in the hospital environment do reveal him as a “user” of the place he uses, often without him knowing, by producing a singular reconfiguration of what is established and constituted as a template for welcome and care. Consequently, the authors review this reconfiguration of the medical institution through observation of various times when a specific interactive dynamics is put into effect between the patient and the other patients but also with the nursing body and its hierarchy. They suggest the idea of deploying a primary fantasy scenario peculiar to the patient and perceptible in the movements and feelings of the professionals, in the actions of this patient and in his statements during dual conversations. The known traumatic elements of the patient's life are put into correspondence with the scenes produced in the institutional space. These mappings show how the hospital can be the theater of a simple repetition of the traumatisms or become a convenient space for their partial elaboration.
