Abstract
The lateral approach to the disk is an effective means for accessing a far lateral disk herniation compressing the exiting nerve root. A typical posterolateral approach does not provide adequate access to a far lateral disk herniation without removing a significant amount of facet joint and pars interarticularis. By approaching from the “outside-in,” the far lateral disk herniation is more accessible with less bony removal.
