► National study assesses the effect of comprehensive state tobacco control programs. ► Data are for 1991-2006 from the Behavioral Risk Factor Surveillance System. ► Contemporaneous and cumulative measures of program expenditures are tested. ► These programs reduce prevalence of adult cigarette smoking adults. ► These programs are a cost-effective type of public health intervention.
