Abstract
⺠An ontology for the health care of chronically ill patients at home is introduced. ⺠The ontology is extended with intervention plans for 19 chronic diseases. ⺠A method to personalize the ontology and the intervention plans is implemented. ⺠A MDSS uses the personalized ontology to help clinicians to make decisions. ⺠Tests confirm clinicians' satisfaction on these tools in controlled real settings.
