► CAA is an age-associated disease with amyloid deposition in brain vessel walls. ► CAA causes mechanical and non-mechanical complications. ► CAA non-mechanical cerebrovascular dysregulation are common and more deleterious than mechanical. ► Aβ 1-40 disrupt cerebrovascular regulation via endothelial-dependent factors. ► The non-mechanical changes may explain cognitive decline in CAA.
