► Random pattern skin flaps are a very important tool in plastic and reconstructive surgery. ► Delay procedure is effective method to improve the blood flow of skin flaps. ► In this study, FLPI was used to observe the blood flow changes for real-time in rat model. ► FLPI can be an important method to judge the vascularity of the flap.
