Abstract
												Of the 5310 UD observed, 35% contained all the required information in a fully legible manner, 49% were partially legible and in 16% the information was missing. It was found that incomplete identification was responsible for an error rate of administration of 0.15-0.35%, which represents 300 to 700 serious adverse events and an estimated annual overcost of 1,200,000 to 3,600,000 Swiss Francs (1,000,000-3,000,000 Euros) for our 2000-beds hospital.
