Abstract
To secure procedure to monitor drug prescription in both in- and outpatient's prescription with a renal insufficiency (RI), a health-care team including nephrologists and hospital pharmacists has been set up. Formations for community pharmacists and general practitioners to explain the network in order to assure safe and rational use of drugs in RI in both in- and outpatients were set up. Health-care team explained network, presented examples of drugs that require renal adaptation and exchange experience with both community physicians and pharmacists. Improving communication between hospital and community physicians or pharmacists has been shown to be the most effective way of preventing discrepancies in drug treatment for both in- or outpatients. Moreover, this process can be evaluated to see the benefit of the network to secure prescription for patients with a RI.
