Arthrofibrose nach biologisch augmentierter Naht des vorderen Kreuzbandes – was wissen wir?

ZusammenfassungDie arthroskopische Naht des vorderen Kreuzbandes (VKB) mit biologischer Augmentation gilt als eine vielversprechende Alternative zur Behandlung von Patienten mit VKB-Ruptur.Allerdings zeigen sich in ersten Berichten in ungefähr 20% aller Patienten eine Einschränkung der Beuge- und/oder Streckfähigkeit des Kniegelenkes im Sinne einer Arthrofibrose.Der beschriebene Fall illustriert das Problem der Arthrofibrose nach VKB-Naht und stellt wichtige diagnostische Schritte und die Therapie dar. Hauptrisikofaktoren für eine Arthrofibrose sind eine zu weite Indikationsstellung, ein zu früher Operationszeitpunkt als auch die Operation an sich.

SummaryThe primary suture repair of the anterior cruciate ligament (ACL) and biological augmentation is a promising treatment option for patients with ACL tear. However, in approximately 20% of patients limited flexion and/or extension due to arthrofibrosis has been observed.This case illustrates the problem of an arthrofibrosis after arthroscopic ACL suture repair. Diagnostic and therapeutic issues are discussed. Major risk factor for development of an arthrofibrosis are extended indications, a too early surgery and the surgery itself.
