ZusammenfassungAchillessehnenbeschwerden spielen in der Sportorthopädie eine große Rolle. Die Differentialdiagnosen dazu sind sehr breit. Eine häufige Ursache für Achillessehnenbeschwerden insbesondere bei Langstreckenläufern ist die Haglund-Exostose mit Bursitis subachillea. Trotz guter konservativer Therapiemöglichkeiten ist vor allem bei Spitzensportlern eine chirurgische Therapie oft notwendig. Es gibt zwei etablierte offene Operationsvarianten, die Cheilektomie und die Kelly-Keck-Osteotomie sowie die noch wenig etablierte endoskopische Variante.Die bisherigen Daten über das Outcome und den Return to Sport der beiden offenen Operationsformen lassen weder klare Vor- oder Nachteile noch eindeutige Indikationsstellungen erkennen, sodass der Therapieentscheid schlussendlich eine individuelle Frage bleibt. Der vorliegende Artikel gibt eine Zusammenfassung über die Pathologie und deren Therapieoptionen.

SummaryAchilles tendon disorders play a major role in sports orthopaedics. The range of the differential diagnoses is very broad. A common cause for Achilles tendon disorders is the Haglund-Exostosis in combination with Bursitis subachillea, this especially in long distance runners. There are many effective conservative treatment options. However, a surgical therapy is often indicated, particularly in elite track athletes. There are two well-established open surgical methods, the resection of the superior prominence oft the calcaneal tuberosity and the Kelly-Keck osteotomy, as well as the not yet much established endoscopic method.The previous results of the two open surgical methods concerning the Outcome and the Return to Sport neither show evident advantages or disadvantages nor explicit indications. Therefore, the decision which therapy is the best, remains an individual question. The following review article is a summary about the pathology and their treatment options.
